for Continuing Education must be submitted together with the following documents:

1. Profile Company/Institution

2. Record of Financial Status (Bank Statement ofthe recent 3 years)

3. UMCCed New Collaborative Programme Application Form

4. Form Evaluation Course New

5. Commitment Deposit (RM2000 deposit payment within 2 weeks after approval)
